Under the terms of the shared care agreement which your GP practice recently signed and returned to us, you should commit to annual review in The ADHD and Autism Clinic. Please sign and submit the form below to receive confirmation of your first annual review appointment.
Provided you remain stable on your ADHD medication at the current dose you will continue to be reviewed annually by one of our psychiatrists. In the meantime, if you or your GP have any concerns relating to your medication or your ADHD, you should contact the office to arrange an earlier review appointment.
